When I start a new title, I have a bit of boilerplate, etc. for the project. How do I create a new project or document template? I see the option to Save As… .pav Template, but where do I save it and how does it get read by the system?

You can set the location for User Templates by selecting ‘Options’ → ‘Preferences…’ and navigating to ‘Paths’ under the ‘Documents’ tab. The decault location is “Documents\Papyrus Author Documents\Templates”, and if you save a .pap file there, it will be available as a template in the Start Menu of Papyrus Author.
